FUNCTIONAL DISORDERS OF THE BILIARY TRACT IN CHILDREN WITH EROSIVE AND ULCERATIVE LESIONS OF THE GASTROINTESTINAL TRACT

Abstract:

Introduction. Biliary dysfunction is a clinical symptom complex that develops as a result of kinetic- tonic dysfunction of the gallbladder, bile ducts and their sphincters without signs of organic damage . The prevalence of functional disorders of the biliary tract reaches, according to various authors, from 25 to 90% in the structure of diseases of the gastrointestinal tract in children. The purpose of the study is to study the prevalence of functional disorders of the biliary system in erosive and ulcerative lesions of the digestive system.

 

Materials and methods. The study was conducted from 2015 to 2022 at the gastroenterology department of the National Center for Maternal and Child Health and the gastroenterology department of the Osh Regional Children's Hospital. Patients with erosive and ulcerative diseases (EUD) of the gastrointestinal tract were selected using a continuous sampling method. A total of 2249 patients were examined, of which 1643 case histories were analyzed retrospectively. To diagnose pathology of the biliary tract, the clinical picture and ultrasonographic examination of the gallbladder and biliary tract with determination of function were studied. Results . More than 50% of children with erosive and ulcerative lesions of various parts of the digestive tract had clinical symptoms of gallbladder irritation. At the same time , an ultrasound examination of the biliary system revealed 414 more children with dyskinetic manifestations of the biliary system than those with manifestations identified during an objective study. In the control group of children with dyskinetic manifestations there were 43.4%, that is, an order of magnitude less than in the group of children with inflammatory-destructive lesions of the mucous membrane of the digestive tract. In all study groups , a hypotonic-hypokinetic form of dyskinesia was mainly identified .

 

Conclusions. These studies clearly demonstrate the high association of the presence of functional disorders of the biliary system in children with inflammatory diseases, which in this case is confirmation of a violation of autonomic regulation and due to visceral-visceral reflexes of the digestive system in erosive and ulcerative lesions of the gastrointestinal tract.
